ABSTRACT

Objective:To understand and explore the incidence and influencing factors of malnutrition in patients with inflammatory bowel disease. Methods:This study was a cross-sectional study. Patients with inflammatory bowel disease hospitalized in the Department of Gastroenterology of a tertiary hospital in Shenzhen from March 1 to August 31, 2021 were enrolled. Indicators related to nutrition and clinical outcome were collected, including height, weight, body mass index (BMI), nutritional risk screening (NRS 2002) results, malnutrition diagnosis as per Global Leadership Initiative on Malnutrition (GLIM) criteria and Pittsburgh Sleep Quality Index (PSQI). Chi-square test, t-test and Wilcoxon rank sum test were used as appropriate for univariate analysis, and binary logistic regression analysis was used for multivariate analysis. Results:A total of 188 patients were included in this survey. There were 145 (77.1%) patients with no malnutrition, 38 (20.2%) with moderate malnutrition, and 5 (2.7%) with severe malnutrition according to GLIM criteria. In the subgroup of 47 ulcerative colitis patients, 12 (25.5%) were with moderate malnutrition and 3 (3.4%) were with severe malnutrition. In the subgroup of 141 Crohn's disease patients, 26 (18.4%) were with moderate malnutrition and 2 (1.4%) were with severe malnutrition. When divided by the presence or absence of malnutrition, there were statistically significant differences in age ( t = -2.237, P = 0.026), disease stage ( χ 2 = 22.299, P < 0.001), history of digestive tract resection ( χ 2 = 6.890, P = 0.009), intestinal infection ( χ 2 = 4.010, P = 0.045), gastrointestinal symptoms ( χ 2 =11.884, P = 0.001), hemoglobin ( t = 5.160, P < 0.001), serum albumin ( t = 3.96, P < 0.001), prealbumin ( t = 5.061, P < 0.001) and PSQI scores ( t = -4.744, P < 0.001). Multivariate analysis showed that the stage of disease, history of partial resection of digestive tract, gastrointestinal symptoms, hemoglobin, prealbumin and PSQI scores were the main influencing factors of malnutrition. Conclusions:IBD patients at older age, at active stage, and with history of partial digestive tract resection, intestinal infection, gastrointestinal symptoms, low hemoglobin, low serum albumin, low prealbumin and poor sleep quality are more likely to develop malnutrition. Timely intervention should be carried out to improve the nutritional status of these patients.

ABSTRACT

Objective:To observe the changes in brain magnetic resonance imaging (MRI) in patients with major depressive disorder (MDD) after modified electroconvulsive therapy (MECT) relative to before treatment. Methods:A total of 105 patients with MDD who received treatment in Hangzhou Seventh People's Hospital and Sir Run Run Shaw Hospital Affiliated to Zhejiang University School of Medicine from June 2017 to June 2019 were included in this study. The brain MRI data pre- and post-MECT were collected. These patients were divided into study group (abnormal brain structure, n = 51) and control group (normal brain structure; n = 54) according to brain structure pre-treatment as shown on MRI. Clinical efficacy, scores of the Hamilton Rating Scale for Depression and Hamilton Rating Scale for Anxiety pre and post-treatment, and the incidence of complications were compared between the two groups. Results:MRI data revealed that 51 patients had abnormal brain structure, including 16 patients with hippocampal atrophy, 18 patients with brain volume reduction, 10 patients with intracranial cyst, and 7 patients with large occipital cistern. These patients had no obvious changes in brain structure after MECT compared with before MECT. The age of onset was lower in the study group than in the control group [(24.15 ± 1.64) years vs. (29.33 ± 2.71) years, t = -7.751, P < 0.05]. The proportion of female patients (57.4% vs. 47.1%), the proportion of patients with a family history of MDD (81.5% vs. 56.9%), and the proportion of patients with psychotic symptoms (55.6% vs. 35.3%) were significantly higher in the study group than in the control group ( χ2 = 4.96, 7.50, 4.33, all P < 0.05). Multivariate logistic regression showed that family history was a factor unrelated to MDD ( P = 0.997). Abnormal brain structure in patients with MDD was negatively correlated with age ( OR = 3.89, 95% CI = 2.083 - 7.281, P < 0.01) and it was positively correlated with sex and psychotic symptoms ( OR = 12.05, 0.08, 95% CI = 2.063 - 70.439, 0.010 - 0.698, both P < 0.05). The Hamilton Rating Scale for Depression and the Hamilton Rating Scale for Anxiety scores decreased after MECT in both groups (both P < 0.05). The proportions of patients having dizziness/headache (50.98% vs. 27.78%), vomiting (43.14% vs. 22.22%), muscle soreness (29.41% vs. 11.11%), delirium (23.53% vs. 7.41%), and memory impairment (64.71% vs. 33.33%) were significantly higher in the study group than in the control group ( χ2 = 5.93, 5.24, 5.49, 5.27, 10.33, all P < 0.05). Conclusion:Most patients with MDD have an abnormal brain structure. Female patients at a low age of onset with psychotic symptoms tend to have an abnormal brain structure. MECT can greatly improve depressive symptoms and has no obvious impact on brain structure. Patients with MDD who have an abnormal brain structure are more likely to have complications.

ABSTRACT

Based on the urban residents' basic medical insurance system in Qingdao, with hospitals, community aging care institutions and families as the carriers,Qingdao city explored and implemented the long-term medical care insurance system(LTMCI),and established mechanisms for fundraising, payment, handling, service and supervision. The city also designed a scientific application and reimbursement process.Three service modes are designed and put into practice as follows:Long-term care in designated nursing homes or pension institutions; Medical care at designated hospitals; and home health care. These services are charged by quota-lump per bed per day, with the reimbursement ratio of 96%,90% and 96% respectively.A practice over a year proved the LTMCI as effectively alleviating disease burden of patients, and guiding long-term care to serve communities. This way a new type of medical care system comes into being, with medicine-care-recovery-endowment for disabled population.

ABSTRACT

A serum method for evaluating therapeutic efficacy of acute leukemia has been built by magnetic beads-based biological mass spectrometric technology. After analysis of quality control sample, the reproduci-)bility) of within-run assay and between-run assay was obtained and consistent with the previous reports. This result) manifested that the method used for serum peptides analysis was stable and reliable. Serum samples from 30 patients with primary acute leukemia and those with complete remission after treatment were extracted by metal-chelated magnetic beads and detected by MALDI-TOF-MS. Peaks at the m/z range 1000-10000 were analyzed by FlexAnalysis 2.4 software and Biosun_MS software. Approximately 27 differential peaks that were of statistical significance(p<0.05) were obtained. Ten peaks were up-regulated) and 17 peaks were down-regulated) after complete remission, respectively. The diagnostic model based on these differential peaks achieved) 100% sensitivity and 90% specificity. The results indicated that the model based on magnetic beads extraction and MALDI-TOF-MS detection acquires high sensitivity and specificity. Further sequence identification of differential) peaks is useful for evaluation of therapeutic efficacy and mechanism research on acute leukemia.)

ABSTRACT

Objective To design a new method of posterior wall reconstruction for severely corn-minuted and obsolete fracture of the posterior wall of the aeetabulum. Methods (1) Biomechanical study (from two perspectives: the stability and the stress distribution on the femoral head and the acetabu-lure) : six fresh adult' s pelvis and femur specimens were collected and divided into experimental group ( consisted of the left sides) and control group ( consisted of the right sides). After the defects of the pos-terior wall were made, the defects were anatomically reconstructed with the harvested ilium by acetabular tridimentional memory fixation system (ATMFS) in the experimental group, whereas the acetabular frag-ments were repositioned to the defect sites and fixated by the plates and screws in the control group. Lon-gitudinal and latitudinal displacement, stability of the anatomically reconstructed posterior wall of the ace-tabulum, the fitness of femoral head to the acetabulum after anatomical reconstruction and mean and maxi-mum pressure of anterior wall, posterior and cupuh of the acetabuhm were analysed and compared. (2) Cinical study: The posterior wall defect on the harvested ilium piece was repaired with 3-directional loc-king of ATMFS in 10 male patients (at age of 16-50 years, mean 36.4 years) from January 2000 to June 2002. There were seven patients with flesh acetabular fractures and three with old acetabular fractures. The period from fracture to reconstruction for the patients with old fractures was 58-251 days ( 137.7 days an average). The patients were foUowed up for 5.8 years (average 5.2-7.1 years). Results (1) Biomechanical study showed no statistical difference upon stability and fitness of femoral head to acetabu-lum after reconstruction in experimental and control groups. (2) Clinical study: postoperative X-ray film showed no repeated displacement of the fracture and the patients began out-of-bed movement with weight bearing after 1.6 months (1.2-2.1 months). Heterotopic ossification was found in two patients, with no influence on the joint function. According to the elinieal ranking standard of Modified d' Aubigne and Postal, the excellence rate was 93%. Conclusions Anatomical reconstruction of posterior wall of the acetabulum by using serf ilium with ATMS has good stability and stress distribution of the femoral head and the reconstructed acetabulum in clinical application and offers a new method for treatment of severely comminuted and obsolete fracture of posterior wall of acetabulum.

ABSTRACT

AIM: Nur77 expression can lead to T cell apoptosis, and it is activated by the second messenger calcium. The calcium-responsive elements in the Nur77 promoter are two putative binding sites for the transcription factor, myocyte enhancer factor-2D (MEF2D). In this study, we explored whether MEF2D could be acetylated in Nur77-induced T cell apoptosis and whether MEF2D acetylation was affected by the regulation of Nur77 expression. METHODS: The experiment was performed at Department of Hematology-Oncology in the First Hospital of Jilin University from November 2006 to September 2007. ①Escherichia coli DH5?and plasmid pcDNA3 were held by the Research Center in the First Hospital of Jilin University. Plasmids pET32M, Flag-p300 and pSilencerTM-p300 RNAi were gifts from Professor Zhengguo Wu from Hong Kong University of Science and Technology. Plasmid NFATp was contributed by Yun Chen from Harvard University. Jurkat cells were provided by Department of Hematology- Oncology in the First Hospital of Jilin University. ②The full-length Nur77 gene, generated by PCR, and the Nur77-dependent luciferase reporter gene were subcloned into plasmid pcDNA. The variant sequences of MEF2D (1-514, 1-121, 1-300, 301-514) and the MEF2D (4KR) lysine-to-arginine mutants, at lysine sites K245/K250/K267/K279, had the Flag epitope at their amino termini. All of the above plasmids were constructed in bacterial expression vectors and the constructed clones were verified by sequencing and then transfected with liposome DMRIE-C. ③The impact of p300 on the trans-activation function of Nur77, mediated by MEF2D, was detected by luciferase reporter assays after Nur77-dependent reporter gene was transfected together with MEF2D, p300 or NFATp into Jurkat cells. In vivo acetylation of endogenous MEF2D and whether acetylation of MEF2D was affected by blocking the expression of p300 were detected by immunoprecipitation. The acetylated sites of MEF2D were detected by acetylation assays in vitro. The impacts of both MEF2D acetylation defects on trans-activation of Nur77 and on apoptosis of T cells induced by Nur77 were detected by luciferase reporter assays and flow cytometry, respectively. RESULTS: ①Although the dimeric complex of MEF2D and NFATp could not induce the transcription of Nur77, significant transcription was achieved with the ternary complex of p300, NFAT and MEF2D. The dimeric complex of p300 and MEF2D could also significantly induce Nur77transcription.②MEF2D was acetylated after the calcium signaling pathway was activated by PMA/Iono. The acetylation level of MEF2D was markedly reduced when p300 expression was blocked by p300 RNAi. ③The simultaneous mutation of K245/K250/K267/K279 lysine sites largely prevented the acetylation of MEF2D, which demonstrated that K245/K250/K267/K279 lysine sites in vitro were the major sites of MEF2D acetylation. ④The MEF2D acetylation defect increased by 2.48 fold the transcription f of Nur77, when compared with vacant vector. But compared with wild type MEF2D, transcription of Nur77 was decreased by 70.4%. ⑤When compared with vacant vector, Nur77 expression could increase total T cell apoptosis of early and late periods from 4.3% up to 16.3%; the co-expression of wild type MEF2D and Nur77 could further increase total T cells apoptosis of early and late periods from 16.3% to 31.0%. However, the co-expression of acetylation defective MEF2D mutant and Nur77 significantly decreased total T cells apoptosis of early and late periods from 16.3% to 9.2%. CONCLUSION : p300 has a major impact on the transactivation function of Nur77 mediated by MEF2D. MEF2D could be acetylated in Nur77-induced T cell apoptosis and acetylated MEF2D could promote T cell apoptosis by upregulating the transcription function of Nur77.

ABSTRACT

@#: The management of peripheral nerve injury is a tough problem clinically.Intensive studies in the past were focused on the bridging of nerve defects and the improvement of regeneration rate. But actually the clinical results of functional recovery after peripheral nerve lesion is mainly decided by the accurate regeneration of axons to their original target tissues and structures. Therefore, better clinical results could be obtained by a greater understanding of the cellular and molecular biology of selective nerve regeneration and the application of this theory clinically. This paper summarized recent studies on the cellular and molecular biology mechanisms of peripheral nerve selective regeneration.
